Nowadays the mitzvah of Sefirat Haomer is derabanan. Nevertheless, the blessing still says "vetzivanu" implying that Hashem commanded us to count. This is not a problem because the Torah instructs us to obey the Hachamim, and so the Torah has indirectly commanded us to count as well. One must understand the words he's saying when counting. A person who does not understand Hebrew must repeat the count in another language that he does understand. If one started the blessing to count the Sefirat Haomer while incorrectly under the impression that it's the third night and then he realized its really the fourth night for example, he should not say a new beracha.
