When saying the me'en shalosh (after beracha) for wine or grape juice, the proper pronunciation is "al hagefen ve'al pri hagefen." There are some who say "hagafen" but that is not our custom and one should not say it that way. One who accidentally said al hamihya on wine has fulfilled his obligation and does not say thereafter say al hagefen.
