When is it true that the Kohen is over 3 negative commandments for not going up? Only if he was actually in the Shul when the Hazan said "Kohanim" or if they told him to go up or to wash his hands, otherwise he's not over anything. Therefore, a Kohen who is not ABLE to go up for some reason -sick etc. - should leave the room before the Hazan says "Kohanim". And even though technically he's allowed to come right back in, still he should stay out until they're done in order so that people shouldn't say that he isn’t fitting.

A Kohen who is unfit to bless because he married a divorcee, he is a halal, or because of any rabbinic prohibition, is NOT required to leave the Shul when the Hazan says "Kohanim". The reason is because when the Hazan calls up the Kohanim, he obviously means only those that are fit. Some say that even if there is no Kohen present except for the unfit one he still does not leave. However, Hacham Ovadia writes that nevertheless it's better if he does leave. As an aside, he also writes that the Hazan should have in mind when he calls up the Kohanim that he is only calling up those that are fitting.

What if a person had a situation where he was not able to pray Shaharit with a minyan and therefore he missed Torah reading and Bircat Kohanim, and now he has a choice to hear one or the other? Some say he should go hear kriyat hatorah but since it's only an obligation on the tzibur and not on the yahid and some say that it's better to hear the Bircat Kohanim since as we mentioned that might be a mitzvah even on the individual Jew making it a safek mitzvah de'oraitah. There are poskim on either sides of this mahloket.

If a person is in a minyan and there are no Kohanim present, he is allowed to go join another minyan in order to receive the blessing. This is because as we said maybe he has a mitzvah to be blessed by the Kohen. However if he has a role in the minyan (Hazan or he's giving a devar torah etc.), then he cannot go.

Nesiyat Kapayim is not done without a minyan since it's considered a davar kedusha. The Kohanim are allowed to be part of the ten who make up the minyan because they are included in the beracha. A katan however, cannot be counted -just like a katan can't be counted as part of the minyan to say the hazara or kaddish.

If there were ten people in the Shul at the start of the hazara and then some of them left, leaving most of the minyan the Hazan continues with the hazara but the Kohanim cannot do the beracha since there is no minyan at the start of the beracha. But if there WAS a minyan at the start of the beracha and THEN some of them left, the Kohanim should not stop but they should continue as long as there is still at least most of the minyan left.

Even if the Kohen washed his hands in the morning, he still must wash his hands before going up and he must wash until his wrist. He does not wash his own hands; rather the Levi pours water on his hands for him. The Levi must wash his hands before washing the Kohen's hands. The Rema says that we are noheg for the Leviim to rely on the morning washing and not to wash again.

Some say that the Bircat Kohanim is only done when there is a Sefer Torah in the Shul; however most poskim say that even if not it's still okay. Therefore, in a Beit Ha'avel for example, the Kohanim would still go up and bless. (Regarding the Avel himself during the Shiva, if he is a Kohen, he must step out of the room when the Hazan says “Kohanim”. See 182:99)

A Yisrael that does the Bircat Kohanim is over an Aseh. This isur is only talking about someone doing the blessing as a Kohen would. However parents or Rabbeim that bless their children or students with these words are allowed to, and they are even allowed to put their hands on the person's head during the blessing and there is no reason for them to be makpid.

Just as in the Beit Hamikdash the Kohanim had a hiyuv to bless all the Jews once daily (after the morning Korban Tamid), so too the Kohanim today have the same hiyuv- to bless Israel once a day. Therefore, if a the Kohen already did the beracha once that day, he does not have to go up and do it again even if they told him to go up, since he already fulfilled his hiyuv.

Once the Kohen did Nesiyat Kapayim, he finished his hiyuv, however if he wants he can to do it again in another minyan (or if he did it in Shaharit he can do it in Musaf), even if there are other Kohanim there who can do it. Even though now it's a reshoot he is allowed to make another beracha (...Asher kidishanu bekedushato shell Aharon), just like Tefilin or Tzitzit.

If Kohen that hasn't prayed yet walks into a Shul and finds them up to Retzeh (but not after) in the hazara, his lack of prayer does not come in the way of his requirement to bless. Therefore, he should go up. If he hasn't yet said the bircat hatorah he should do so quickly while waiting to bless, if he can't its okay.

If there are at least two Kohanim in the minyan, before they start to bless the sheliyah tzibur calls out: "Kohanim". The Kohanim are only required to bless if they are called up since the pasuk says "emor lahem" which is translated to mean that "to a pair you shall say" this "say"ing is referring to when the shaliyah tzibur says it.

When the Kohanim are blessing, the tzibur must (stand if they are able and) listen and pay attention to the beracha. They should not be saying any pesukim etc. during that time. Just like if a master was blessing his servant wouldn't the servant take great care to pay attention to every word of the blessing? Therefore one should take care not to do anything to take his mind off the beracha.

If the shaliyah tzibur makes a mistake and he skips part of the blessing and the Kohanim go along with it, if he remembers during Sim Shalom then he goes back to the blessing and does it again (with Kohanim). Otherwise, he should do it wherever he remembers. Either way the Kohanim should not make a beracha before they bless. (just as you may not add to the Torah you may subtract, that's why we go back here.)

We do the bircat Kohanim with any tefilah in the day except for Minha and Arvit. We don't do it during Minha because we're worried that maybe the Kohen is drunk, therefore on a fast day that does not have Ne'ilah we would do it in the Minha. The reason why we don't do it at night is because the pasuk says "ad HAYOM hazeh" which is applied to Kohanim.

The reason why the Kohanim can bless on a fast day during Minha is because there is no reason to be worried that they're drunk since they're fasting. Therefore, a Kohen who is not fasting is not allowed to bless during Minha. On a fast day that is not mandatory (7 Adar, Shovavim, etc.) the bircat Kohanim is not done during Minha even if there are ten people there who are fasting.
If the Kohanim already went up though, they shouldn't come down.
